Chronic Disease Clinics
Our doctors and nurses are committed to improving the health and awareness of diabetes in our community. If you are identified as having diabetes, we invite you to participate in our optional Diabetes Cycle of Care.
During these clinics our diabetes nurse will discuss your history and advise of the best ways to manage your condition, formulate a plan in conjunction with your GP to ensure your diabetes is optimally managed and the risk of complications are minimised.
Diabetes Cycle of Care appointments are bulk billed with no out of pocket cost for all patients.

The life program is a FREE program that is ran fortnightly for 8 weeks. During this time we discuss lifestyle changes that can dramatically reduce your risks of developing type 2 diabetes, heart disease and stroke.

For any patients who experience chronic illness such as heart disease, arthritis, kidney disease or any other condition that lasts for more than six months, we offer a supportive nurse-led clinic to ensure your care is optimal.
In conjunction with your GP, you will receive a written care plan outlining your health goals, activities and other medical or allied health specialists who are involved in your care.
Chronic Disease appointments are bulk billed with no out of pocket cost for all patients.
Health Assessments are provided at our clinic year-round, with the most common assessments being for over 75s and patients aged between 45-49 years of age.
