CARE PLAN/CHRONIC DISEASE
For any patients who experience chronic illness such as heart disease, arthritis, kidney disease or any other condition that lasts for more than six months, we offer a supportive nurse-led clinic to ensure your care is optimal.
In conjunction with your GP, you will receive a written care plan outlining your health goals, activities and other medical or allied health specialists who are involved in your care.
Chronic Disease appointments are bulk billed with no out of pocket cost for all patients.
For each calendar year you will receive 5 vouchers to use. If you do not use all 5 vouchers they will not carry over to the following year.
HOW LONG WILL MY CDM APPOINTMENT TAKE?
CDM appointments usually take between 30 minutes to an hour. This incorporates a consult with the chronic disease nurse and your doctor. Please note that the nurse portion of the appointment may be done over the phone, reception will advise you if this is the case.
HEALTH ASSESSMENTS
Health Assessments are provided at our clinic year-round, with the most common assessments being for over 75s and patients aged between 45-49 years of age.
These assessments provide a structured way of identifying health issues and conditions that are potentially preventable or amenable to interventions in order to improve health and/or quality of life.
